Snowmaker, we've already mentioned "Somewhere in Time" and "Spinal Tap".
Anyway, I'll add this cool homage...
The Kinks (1965)

Sleater-Kinney (1997)

I'll pick some others then:

Let me just point out that I don't like that last one for what it depicts. I just find it amazing (for lack of a better word) that these guys had made this cover for their CD just a few months before 9/11.

My mistake, sometimes my brain takes a vacation and forgets to tell me.
I remember a Megadeth poster my roommate had in college, where Vic Rattlehead was dressed up like a general. I think it was tied to the "Rust in Peace" album (it had just come out). That was back when metal bands took the time to have cartoon mascots.

In light of the Catholic rape/cover up drama of late...this cover's time has come.
I have never heard "CANNIBAL CORPSE" but their covers are really cool if you're into GRAPHIC ZOMBIE/LIVING DEAD stuff. You should see the back covers
This cover is rather compared to their others

Too bad I'm too handicapped to know how to add images but mine would have to be:
Quadrophenia
Stereophonics: Performance and Cocktails
(As shown below... Thanks Snowmaker)

I knew if I looked hard enough, there'd be a pre-existing thread for this! Two favorites of mine:

Tim McGraw - Set This Circus Down (2001)

Norah Jones - Not Too Late (2007)

Wow. 8 year bump. Who says search function sucks.
There was an artist named Guy Pellart. Put out a book of rock n roll portraits around '73-74. Then in 1974 did the only two album covers he ever did, my two favorite covers.
Bowie-Diamond Dogs
Rolling Stones-It's Only Rock n Roll

Not a true album cover, but my favorite gatefold band picture ever:

It looks like it's about 3AM on a hot summer night, back when NYC was still gritty as hell.
Staying with actual album covers, a few good ones:

I love the color blocks in the corner on that one. It's like they're saying that this masterpiece of a painting is actually a mass-produced print.

I know the guy's a Nazi loser scumbag, but that's the coolest metal cover of all time. It helps that it's a hundred year old woodcut.

I love the clean, graphical look of that cover.

Roxy Music always had great covers and this is my personal favorite off of their most underrated album. It's creepy and glamorous at the same time, reflecting the music.

This one takes some explanation... Agreed, this cover is truly awful from most perspectives. On the other hand, to me it's a perfect artifact of the late seventies when I was a little kid. The little hearts on the letters, the too-colorful "renaissance faire" image with the blown dried lion-mane hair. There's a fucking goat down there in the corner. I could see someone's older sister having a poster off it on their wall in about 1979 behind the shelf showing off their Breyer horse collection. I can't help but love it. It helps that's it's a pretty damned good album.
Two more recent ones I've liked:

This one reminds me of the "arcologies" in "Neuromancer", but with blue skies. I just like the image.

It looks like something someone would draw in marker on the inside of a Trapper Keeper. Also, I like anything to do with ents.

The throwback look of this one really grabs me:

Something about this one grabs me:

This one is a classic:

This one reminds me of a modern take on a Durer engraving:
